By typing your name, and paying the $500 fee, you are digitally signing and agreeing to the following terms: This is a prepaid flat fee of $500 for one initial consultation session only (approximately 50 minutes) via phone or video. The consultation is limited to general legal information and discussion of your legal questions. Important: Paying this fee and attending the consultation does NOT create an attorney-client relationship. An attorney-client relationship will only be formed if we both agree to it in a separate written engagement agreement after the consultation. The $500 fee must be paid in full before the consultation is scheduled. This prepaid fee will not be deposited into my lawyer trust account. You may cancel or terminate this arrangement at any time for any reason (or no reason). If the consultation services for which the fee was paid are not completed, you may be entitled to a refund of all or part of the fee. Refunds will be calculated as follows: Full refund if you cancel with at least 24 hours advance notice; Otherwise, a reasonable refund based on the value of any preparation time or services already provided. Acknowledgement and signature: By checking this box and submitting payment, you confirm that you have read and understand all of the above terms, including that the fee will not be deposited into a lawyer trust account and that you may be entitled to a refund (calculated as described) if the services are not completed.
